In my opinion all of your grading is fair, I would only quibble with 2018. I would probably give that year a B (I realize this is totally subjective).

That would give Harbaugh an overall grade of 3.9 according to my grading (again given that the AD has decided this year doesn't count).

I realize that there’s no chance of convincing you, however I want to share this story anyway. Because, yes, irreparable damage can be done by not going to school. 

 

My bosses daughter has been struggling with depression/anxiety for several years, she’s 16 years old. Last year she changed schools and was finally doing well. She was doing so well in fact that she was able to stop her antidepressants. 
 

Then comes Covid in the spring. She had to stop going to school stop her extracurricular activities and she began to backslide. They had to place her back on her medicine within a few weeks of school closing. 
 

Fast forward six more months and this beautiful young girl attempted suicide. Luckily, it was an attempt and not a success. She had to be placed in inpatient psych care for a month. She had to withdraw from school this year for treatment. And they just moved her to Utah (from Texas) to a facility for treatment where she will reside for 6-9 months. 
 

I realize this is an anecdote and not generalizable to the population at large, but don’t tell me there isn’t collateral damage to kids being taken out of school.

And payments for hips and knees continue to get slashed  they are such a large line item on Medicare’s budget that it’s easy to look at them and say “oh look we do 500k knee replacements a year and pay $800  lets cut that to $600 and save a shit load of money”

 

 

You’re not misunderstanding…

You’re not misunderstanding the biology. I think you’re misunderstanding the clinical question. 

Beaumont is testing everyone as a sample to see how many of their workers have antibodies. They will then use that data to extrapolate various things. They are not using the test to see if someone was infected during that shift. 

They are jus checking how many asymptomatic or minimally symptomatic people there are. They are also giving employees hope that if they have antibodies they are immune (we hope) and can work without fear. 

Yes this may miss some people who were just infected but they will likely retest the negatives in 2-4 weeks.

I think what you’re referring to is more that the peak of the curve is behind us. Yes the curve has flattened from initial reports but the decreasing numbers of infected hopefully reflect that Detroit has peaked and will start coming down. 

As others have said in this thread. While NYC and Detroit are peaking now, other areas will peak later. Internal data at my hospital in GR is still showing our peak in late May. Hopefully as we get more data that goes away or flattens further.

Just as the US is driven by NYC (in terms of peak etc), So to is Michigan driven by the Detroit metro area. I’m in Grand Rapids and the hospitals here are running their own data. Based on their modeling we are weeks behind the east side of the state. They don’t think we will peak over here until May-June.  

Things at the hospital have been on lockdown for 3 weeks in preparation but we just aren’t seeing the numbers here yet.  So Detroit will just be coming down right when GR and west Michigan start to ramp up.

Based on the link you provided to the health data information (thanks by the way!) it seems that we have pushed our (US as a whole) peak out to mid April by our social distancing measures. New York and Michigan (along with NJ and Connecticut and a few others) appear to have started social distancing way too late to flatten the curve much. They are within 7-10 days of hitting the peak. 

Big population centers like California Texas and Florida however have successfully flattened their curve it appears and have pushed out their peak dates quite a ways which is why I think the overall peak has moved further away.

The good news with that is that hopefully we will be able to successfully treat more people by delaying and not overwhelm the healthcare system. 

The data is grim for NY, Michigan Louisiana NJ Connecticut etc though.

Full disclosure; I’m an orthopedic surgeon. For you medical professionals you know that means I left my medicine brain behind me long ago. 

My question for those of you more in the know: if this is as contagious as it appears and it has been stateside since at least mid January. How on earth has this not made its way through the majority of population at this point?

in my hospital we have been struggling with capacity issues since January. (This isn’t atypical for us during flu season) and my medicine colleagues have mentioned this being a “bad” flu season. 

We all have stories of having a bad cold or family and friends who had a bad cold this year. Isn't it possible (or even likely) that this was corona?

With only 12% of infections requiring hospitalization and less than 3% the icu (my numbers may be off. I saw these from New York earlier this week) the vast majority of positive tests are likely from people who would have never presented for medical care with their illness had there not been a huge health crisis or would have just seen their PCP. Instead these people are flooding hospitals and ERs and we are utilizing all available resources just to tell them to go home and quarantine. Which is a huge double edged sword  on 1 hand we get much needed info  on the other hand we utilize our scarce PPE resources on people who don’t “need” it  

Obviously more widespread testing is helpful from a policy standpoint. Likewise we would love to be able to get antibody testing up and running to know who has had this already and help to guide quarantine timelines  

None of this is to say we aren’t being appropriate in our quarantine measures, since e don’t know these answers we can’t assume, just some questions I have and some food for thought.   

Anyway, to my ER and medicine colleagues. Keep your heads up and keep plugging along. And keep yourselves safe. We’re gonna need you!  No one wants this orthopedic surgeon managing their lung issues!

I don't think you are reading the terms correctly.  Silver medallion members get 7 miles per dollar vs 5 miles per dollar for non medallion members when spending on delta flights.  For your american express card you will continue to earn at the same rates.  2x miles on Delta purchases and 1x miles on everything else.  You'll definitely get extra miles for your delta purchases. But you won't be changing your delta earnings with every day purchases.  Sorry for the bad news.
