There is a managed scramble for open positions. There is a list of programs with open positions that you and everyone else who didn't match into a program can apply to a certain number on the first day. If one of the programs you applied to wants to interview you they set something up and so on with the other programs you applied to. You and the program submit a rank list and the computer tells you whether there is a match. If no match the process repeats itself with an updated list of the programs from round one that still have open spots. I think there are 3 or 4 rounds of this and by then there is almost nothing left.
If you still haven't secured a spot you basically have to find a job. Which in itself isn't easy as you have a professional degree without a profession... people do research if they can find someone to take them on or one person my wife knew took an admin position at a hospital. There will be spots that open up during the year (people can't get visas in time to start, people get fired, etc.) that you can apply for but this is real longshot.
You can apply and interview the next year but each year you're out of med school it gets successively harder as you're further away from your grad date. There is also the issue mentioned above that there are more med students being churned out each year without a corresponding increase in residencies. Most places would rather take on a fresh grad than someone who failed to match in previous cycles.
The person I mentioned above who took the admin position eventually matched two years later but she was extremely lucky and most people are pretty much screwed after not matching once or twice.

Wow, that's not really kosher. Can't say I'm surprised though. Missouri recently passed a law that will allow non-residency trained MD's to act as physician's assistants (essentially anyway). I think Arkansas has a bill that's being debated for the same thing. As far as I know Michigan has no such law on the books. The doc that hired her is going to get murdered in lawsuit if there is a bad outcome... not to mention if the state board finds out.

Add in that you can be fired at any time for even the most trivial reason during training and your career is essentially ruined. But you still owe back all of that student loan money.
For anyone who doesn't know residency/fellowship is a series of one year contracts that can be non-renewed for just about any stupid reason you can think of... some places have resident unions that can protect you a bit and some places have a strong GME dept that might help but most don't.
We had no idea about anything related to medical training/school before my wife went through it. If you wanted a system that takes potentially compassionate/selfless doctors and crushes all compassion/selflessness out of them this is it! Well done American medical training!

Ain't no way he's going back to Indiana. There was some funny business with his enrollment and he pretty much got screwed. If I remember correctly he showed up enrolled and then took a summer class (or classes) "didn't do well enough" and wasn't given a scholarship. I'm sure it was purely coincidental that Indiana was over the scholarship limit by 1 at the time too.
Anyway, I remember reading about it and that's the outline of the story.

Just be aware that Mt. Olympus is something like a 6 hour hike and Lake Blanche took the wife and I about 5 hours (though I did get off the trail a bit). I was not at all prepared for Lake Blanche as we had just moved here and all we took was a bottle of water each. I was bonking the whole way down and it was bad enough that I went out and bought a hydration pack and hiking shoes the next day. Just make sure you research the hikes before getting yourself in too deep (a lesson I learned the hard way). Nowadays I'd be fine as I am an avid hiker and live at high altitude.
