To be fair, it's not a plot hole unless it never again is addressed in a future episode, which I think is unlikely.
Even then, you could argue that the driver just got the hell out of there during all the chaos.
However, I think it's a fair guess that the driver was the chick at the wall that Andrea was talking to early in the episode. Not much of a leap there.
Hadn't the Governor's crew already left for the prison at that point? That's why Andrea was talking to the people at the wall, to see where they'd gone.
But yeah, my assumption was that the driver of the van just took off back through the gates after getting out of the truck. Why would she have stuck around?

Yeah, I thought it was pretty clear when she (?) ran past Michonne who was just inside the fence. I was a little confused at how they managed to get a guy on the tower though... That's just piss poor security for a veteran crew who know an attack is imminent.
