Medical procedure, yes. Also not a short one. I sat for 12 hour-long sessions over the course of a year to get the one on my back removed - total cost was right around $2500...it was roughly 7x9" in size and what they considered to be a "large" tattoo.
Turns out you CAN wipe way mistakes you made when you were 18, its just a long and very painful process.
