Medical procedure, yes. Also not a short one. I sat for 12 hour-long sessions over the course of a year to get the one on my back removed - total cost was right around $2500...it was roughly 7x9" in size and what they considered to be a "large" tattoo.
Turns out you CAN wipe way mistakes you made when you were 18, its just a long and very painful process.

The best is the "back tattoo age progression" about halfway through. I went to the Guns n Roses show last year, and while the concert was epic, it was obvious that the hot metal chicks from the 80's were "rode hard and put away wet", and the tattoos had "stretched". Or put another way - if they jumped on the hood of Whitesnake's car today, they certainly would cause a dent...
